@psionacles @GunEye I agree with most of your points, however making games hardcore and less casualised doesn't necessary has to do with difficulty and brutality levels: Ingenuity, creativity, mental challenges, open-ended gameplay, freedom: these are the things that make a game unique and thoroughly interesting + story of course and overall graphics and sounds quality. 

Regarding The Witcher 3/2 Insane mode: I actually think its a bad thing. The game is far from bug free, and some trail and error deaths are unavoidable especially at high difficulties. I remember a section late in Act3 in TW2, were a gargoyle fell on top of my head from above, just as it spawned, killing me instantly!!

I played the hardest difficulty (not Insane). Now imagine I was on Insane. That was just a cheap death. Losing by cheap shots isn't fun, and its by no means hardcore. Also you can die from failing on lousy quick time events.
